Childcare Application

By providing the information below, you are indicating your willingness to work as a paid childcare provider for children of Summit Church for special events, classes, or meetings. You must undergo a background check before proceeding if this has not been done already through Summit.

In addition to the below information, you will also be required to complete a W9 form before your first date of service in order to receive your payment. More information will be sent via email.
